As 2022 comes to a close, we would like to take a moment to thank our many Friends, Supporters and Partners who have shared in MyClinic’s evolution to become a leading community resource addressing the Social Determinants of Health (SDOH).
MyClinic’s dedicated Board of Directors and staff are proud to work hard every day to fulfill our MISSION
MyClinic’s team, including volunteers and collaborative partners, is committed to empowering individuals and families who have limited resources to improve their health and quality of life by providing information, access to clinical care and community services.
Our new “North Star” has led MyClinic to become the Health and Wellness Navigation Hub for anyone with limited resources living in North Palm Beach County. During this year, MyClinic has served 4,290+ registered clients and provided 2,001 SDOH Referrals, 1,316 Care Coordination Visits, & 676 Behavioral Health Visits.
MyClinic has many new projects planned for 2023 and we look forward to continuing to grow our services and programs to serve people in need throughout our community!

CARE COORDINATION/NAVIGATION Program
Social Determinants of Health
Social Determinants of Health (SDOH) are circumstances and conditions that influence a person’s overall health. MyClinic’s trained Care Coordinators take time to identify your needs including: food and nutrition support; housing, rent and utilities assistance; transportation to medical appointments; legal assistance; and family support. We will connect you with a comprehensive array of resources and services through collaborations with community partners and guide you and your family as your circumstances change.
Post-Acute Care Coordination
When you or a loved one are recovering from an acute illness, surgery or a hospital stay, MyClinic is here to help make the process easier and less stressful. Our compassionate care coordinators will help you understand discharge instructions and manage simple tasks such as picking up prescription medication, food and other supplies to help you get back on your feet.
BEHAVIORAL HEALTH Program
Mental health and wellness play a key role in overall health. Now more than ever, individuals and families are facing stressful situations and need emotional support. MyClinic’s Behavioral Health Program includes a licensed Mental Health Counselor (Bilingual) who is trained in helping people survive and thrive in these challenging times. Services include: individual counseling for adults and children; family counseling; support groups; coping strategies related to living with chronic diseases and counseling for substance abuse issues.
MEDICAL CARE Program
Navigating the healthcare system can be confusing and difficult. We are here to help connect you with appropriate primary care providers and assist you in the registration process if needed. We may also be able to refer you to medical specialists who are affiliated with MyClinic.
WOMENS HEALTH Program
The Promise Fund of Florida supports MyClinic’s Women’s Breast and Cervical Cancer Healthcare Navigator Program, with the purpose to improve access to breast and cervical health care for women in need of early detection screenings and follow up.
URGENT DENTAL CARE Program
MyClinic maintains a network of dentists in our community who are able to provide urgent dental care right in their office to our registered clients.
HEALTHY LIVING Program
MyClinic will host a variety of wellness programs provided in collaboration with local partners including: healthy eating, cooking and nutrition classes; supplies and education related to chronic disease management; and active living opportunities. Activities will take place virtually or in-person in accordance with current CDC COVID guidelines.
